Side Effects of Hormone Replacement Therapy: What's Common and What's Rare

Patients almost never ask about the side effects of hormone replacement therapy without a second question sitting underneath it: is this going to hurt me? Answering honestly means separating two things that most articles on the subject blur together — the mild, temporary effects that show up while your dose is being adjusted, and the serious long-term risks that made headlines in 2002 and have been re-examined ever since. They are not the same category of problem, and they do not carry the same weight in a decision. Christopher J. Here is what actually happens, how often, and what changes the odds.
Two Different Questions Live Inside This One
The first is about tolerability: what will I feel in the first few months, and will it be worth it? Those effects are common, usually mild, and almost always fixable by changing the dose or the delivery method. Breast tenderness is not a reason to abandon therapy; it is a reason to adjust it.
The second is about risk: does hormone therapy raise my chance of a serious event — a clot, a stroke, breast cancer? That question has a real answer, but only when it is asked with numbers attached and with your age, your health history, and your specific formulation in the frame. A risk figure quoted without those three things is not information. It is a headline.
What Most Patients Actually Experience in the First Few Months
The American College of Obstetricians and Gynecologists lists the common side effects of hormone therapy as vaginal spotting or bleeding that usually stops within six months, temporary breast soreness, bloating from fluid retention, and headaches. That list matches what we see in practice. Most of it appears in the first six to eight weeks and fades as levels stabilize.
A few others are worth naming because patients are often caught off guard by them. Mood can move in either direction during the first month, particularly when progesterone is introduced or changed. Nausea shows up mainly with oral preparations taken on an empty stomach. Skin irritation is common with patches and, less often, at the application site with gels. Breakthrough bleeding in a woman whose periods had stopped is unsettling, but it usually reflects a dose or a progesterone schedule that needs correcting rather than anything ominous — though bleeding that persists past six months, or begins after a long quiet stretch, always warrants evaluation rather than reassurance.
What these have in common is that they are dose-dependent and reversible. That is the practical distinction worth holding onto: an adjustment problem behaves differently from a safety problem.
Side Effects Specific to Testosterone Therapy
Testosterone has its own list. In men, the most common measurable effect is a rise in hematocrit — the concentration of red blood cells — which thickens the blood and, left unmonitored, raises clotting risk. It is also the best single argument for scheduled lab work rather than a set-and-forget prescription. The Endocrine Society clinical practice guideline advises against starting testosterone without prior evaluation in men with a baseline hematocrit above 50 percent, severe untreated sleep apnea, or Class III or IV heart failure. Testosterone also suppresses sperm production, so it should not be prescribed to a man trying to conceive in the near term.
Other effects in men include acne or oilier skin, fluid retention, breast tenderness or enlargement when too much testosterone converts to estrogen, and worsening of existing sleep apnea. Nearly all of them are dose-related.
Women prescribed low-dose testosterone — a legitimate and frequently overlooked part of female hormone care — can experience acne, increased facial or body hair, and rarely a deepening of the voice. The first two resolve when the dose comes down. Voice change may not, which is why women's testosterone dosing should be conservative and closely monitored rather than extrapolated from male protocols. When the Delivery Method Is the Real Problem
A meaningful share of what patients call a side effect of hormone therapy is really a side effect of how the hormone is being delivered. Oral estrogen passes through the liver before reaching the rest of the body, and that first pass drives several of the risks people worry about most. ACOG notes that patches, sprays, and rings may pose less clot risk than pills taken by mouth, and that the small increase in gallbladder disease associated with estrogen is greatest with pills.
Injections produce peaks and troughs, so some men feel irritable or flat as the level falls near the end of a cycle — a scheduling problem rather than a therapy failure. Hormone pellets carry a different set again: bruising, bleeding, or infection at the insertion site, occasional extrusion of the pellet, and the more fundamental issue that once a pellet is placed the dose cannot be reduced if it turns out to be too high. ACOG specifically flags safety concerns about pellet therapy alongside the fact that compounded preparations are not FDA-regulated and can vary in strength and purity.
That variability is not an argument against bioidentical hormones — estradiol, micronized progesterone, and testosterone all exist as FDA-approved bioidentical products — but it is a real argument for knowing which pharmacy is making your prescription and how its work is verified.
The Serious Risks, Stated in Absolute Numbers
The Women's Health Initiative is the study behind almost every alarming sentence written about hormone therapy since 2002, and what it actually found deserves to be quoted precisely. Among roughly 16,600 postmenopausal women aged 50 to 79 taking conjugated equine estrogens plus medroxyprogesterone acetate, the trial reported absolute excess risks per 10,000 person-years of 7 more coronary heart disease events, 8 more strokes, 8 more pulmonary embolisms, and 8 more invasive breast cancers, alongside 6 fewer colorectal cancers and 5 fewer hip fractures.
Eight additional breast cancers per 10,000 women per year is a real increase and should not be waved away. It is also a rate of roughly 0.08 percent per year, measured in women whose average age at enrollment was well past menopause, using a synthetic combination that is no longer the default prescription. Both halves of that sentence are true, and quoting only one of them is how this topic gets distorted — in either direction.
The estrogen-only arm behaved differently. The National Cancer Institute's summary of the evidence describes a lower risk of breast cancer and a lower risk of death from breast cancer with systemic estrogen alone, alongside an increased risk of endometrial cancer when estrogen is used without a progestogen in a woman who still has a uterus — precisely why progesterone is prescribed with it — and an increased risk of dementia when hormone therapy is started at age 65 or older.
Why Your Age at the Start Changes the Math
The most useful variable in this entire conversation is when you begin. Risk is not a fixed property of hormone therapy; it shifts substantially with how close you are to menopause. The Menopause Society, which sets the mainstream position in this field, describes hormone therapy as the most effective available treatment for bothersome hot flashes and night sweats, and the profession generally regards the benefit-risk balance as most favorable for women who start under age 60 or within ten years of their final period.
The mirror image holds as well. Beginning systemic hormone therapy a decade or more after menopause carries a less favorable profile — the dementia signal above comes from exactly that group. Time since menopause is the variable most articles omit, and it changes the answer more than almost anything else on this page.
Route and Formulation Change the Side-Effect Profile
The specific molecule matters too. The French E3N cohort followed more than 80,000 postmenopausal women and compared hormone regimens against each other directly. It found that the relative risk of breast cancer was 1.00 for estrogen combined with progesterone and 1.69 for estrogen combined with other progestins — no detectable increase with progesterone, a substantial one with synthetic progestins, over a mean of about eight postmenopausal years of follow-up.
Two honest caveats belong with that number. E3N is an observational cohort rather than a randomized trial, so it establishes association, not proof. And its follow-up was not long enough to say what happens after a decade of continuous use. What it supports is a preference, not a guarantee of safety: the progestogen component is a genuine decision with genuine consequences, and defaulting to a synthetic progestin because it is what the formulary stocks deserves more scrutiny than it usually gets. What the Newest Testosterone Data Shows About Heart Risk
For men, the cardiovascular question went unanswered for more than a decade. It now has a large randomized answer. The TRAVERSE trial enrolled 5,246 men aged 45 to 80 with hypogonadism and either existing cardiovascular disease or a high risk of it, and found that testosterone-replacement therapy was noninferior to placebo for major adverse cardiac events, with a hazard ratio of 0.96.
The same trial found a higher incidence of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group. That is the complete finding, and both halves belong in a patient conversation. Testosterone therapy in appropriately selected men does not appear to raise heart attack and stroke risk the way many physicians assumed for years — and it is not risk-free, which is why monitoring is part of the prescription rather than an optional extra.
Who Should Not Take Hormone Therapy
Some conditions make systemic hormone therapy inadvisable no matter how well it might otherwise work. For women these generally include unexplained vaginal bleeding that has not been evaluated, known or suspected breast cancer or another estrogen-sensitive cancer, active or prior blood clots in the legs or lungs, a recent stroke or heart attack, active liver disease, and known clotting disorders. For men they include existing prostate or male breast cancer, an untreated elevated hematocrit, severe untreated sleep apnea, uncontrolled heart failure, and a near-term desire for fertility.
Several of these are relative rather than absolute — a clot with a known reversible cause is a different conversation from an unprovoked clot in someone with a thrombophilia — and low-dose vaginal estrogen for genitourinary symptoms is evaluated separately from systemic therapy. The point of the list is not to disqualify people. It is that the screening has to happen before the prescription, not after.
How Side Effects Get Managed Instead of Endured
Almost every common side effect of hormone therapy has a lever attached to it. Breast tenderness and bloating usually respond to a lower estrogen dose. Mood changes on progesterone often respond to a change in dose, a change in timing, or a switch from a synthetic progestin to micronized progesterone. Concern about clot risk argues for a transdermal route instead of an oral one. A rising hematocrit on testosterone argues for a lower dose, a different delivery method, or a pause and re-evaluation.
None of that is possible without follow-up testing on a schedule and a physician willing to change the plan. Patients who tell us their previous therapy "did not agree with them" have often described a dose that was never adjusted after the initial prescription. Getting a Risk Picture That Reflects You
The general side-effect profile of hormone replacement therapy is knowable, and it is written above. Your side-effect profile is a different document. It depends on your age, how long ago menopause happened, your personal and family history of breast cancer and clotting, your blood pressure and metabolic health, which hormone is actually deficient, and which formulation and route you use.
