What Andropause Actually Is

The terminology varies, but the underlying physiology is consistent. Beginning around age 30, testosterone production falls by roughly one to two percent per year, and for many men that cumulative loss eventually crosses the threshold at which the body can no longer function the way it once did.
Testosterone is not only a sexual hormone. It is also important to understand what andropause is not. It is not a universal, inevitable diagnosis for every man over 45, and low testosterone on a single lab draw does not automatically mean andropause. Testosterone varies with the time of day, sleep quality, acute illness, and body composition. How Andropause Differs From Menopause
The comparison to menopause is useful but imperfect, and the differences explain a great deal about why male hormone decline goes untreated for so long.

Research from the European Male Ageing Study, one of the largest population studies of aging men, found that sexual symptoms — reduced libido, fewer morning erections, and erectile difficulty — were the symptoms most consistently associated with genuinely low testosterone levels, more so than the general fatigue and mood complaints that overlap with many other conditions.
Cognitive and emotional changes round out the picture. Brain fog, slower recall, difficulty sustaining concentration, irritability, a shorter fuse, and a low-grade depressive flatness are all commonly reported. These are frequently the symptoms that finally prompt a man to seek help — and frequently the ones most likely to be treated with an antidepressant before anyone checks a hormone level.
Why Andropause Is So Often Missed
Several factors conspire to keep male hormone decline undiagnosed, and understanding them helps explain why so many men spend years without answers.
The first is attribution. Every symptom of andropause has a plausible non-hormonal explanation. Fatigue is attributed to workload, weight gain to diet, low mood to life circumstances, and reduced libido to relationship dynamics or age. Any one symptom in isolation is easy to explain away; only the pattern points to a hormonal cause, and patterns are only visible to someone looking for them.
The second is testing practice. Testosterone follows a daily rhythm, peaking in the morning and falling through the day, so a level drawn in the afternoon can read substantially lower than the same man's morning value. Both the American Urological Association and the Endocrine Society advise diagnosing testosterone deficiency using early-morning measurements confirmed on two separate occasions, precisely because a single convenience draw is unreliable in either direction.
The third is incomplete panels. Total testosterone alone is a poor picture of androgen status. Much of circulating testosterone is bound to sex hormone-binding globulin (SHBG) and unavailable to tissue; SHBG rises with age, which means a man's total testosterone can look acceptable while his free, biologically active testosterone is meaningfully low. Without free testosterone and SHBG in the panel, that man is told his labs are normal.
The fourth is threshold thinking. The American Urological Association identifies a total testosterone below 300 ng/dL as the level defining testosterone deficiency, but reference ranges on lab reports are built from a general population that includes many symptomatic men. A result flagged "within range" is not the same as a result that is optimal for the individual in front of you, and treating the flag rather than the patient leaves a great many symptomatic men without care.
How Andropause Is Evaluated at The Riegel Center

This evaluation also identifies when the answer is not andropause. Sleep apnea, thyroid disease, medication effects, poorly controlled metabolic disease, and depression can all mimic male hormone decline, and some suppress testosterone directly. A thorough workup distinguishes between them, which matters as much as confirming the diagnosis when it is present.
